Before submitting a 4th Attempt Application:
- You must wait at least 30 days from your failed or rejected 3rd attempt submission date.
- You must complete the required Remedial Training Course through PhlebCE.com.
- You must obtain a completion certificate.
- Your completion certificate must be uploaded with your 4th Attempt Application.
Purchasing your exam course does not complete your application.
Every candidate is responsible for completing and submitting their own CPT Application before exam access can be granted.
The CPT 4th–6th Attempt Application & Exam course includes a 30-day access period beginning on the date of purchase.

- Log into the NHCO Portal.
- Select Dashboard.
- Open the CPT 4th–6th Attempt Exam Application.
- Complete all required sections.
- Upload all required documentation, including your Remedial Training completion certificate.
- Submit your application.
Ensure your application is submitted and approved and that your exam is completed and submitted before your 30-day exam access period expires.
What Happens Next?After submission:
- Your application enters the auditing queue.
- Review generally takes up to 5 business days.
- You will receive a notification once the review has been completed.
- If approved, your exam will unlock automatically.
- Your approval message will include exam access instructions.
